Client Consent, Disclosure & Practice Policies
The following information is provided to help clients understand the scope of services, professional standards, and office policies prior to booking.
Informed Consent & Accuracy of Information
Clients are responsible for providing accurate and complete information on all intake and consent forms. By scheduling services, clients acknowledge that massage therapy treatment is provided by a South Carolina–licensed massage therapist.
Personal health information is collected and maintained in accordance with applicable state and federal privacy laws. All information remains confidential unless disclosure is required by law or necessary to ensure safe and effective care.
Medical Conditions & Contraindications
Massage therapy may be contraindicated for certain medical conditions. Clients are responsible for disclosing all known health conditions, injuries, surgeries, medications, and relevant medical history prior to treatment.
In accordance with South Carolina regulations, a written referral or clearance from a licensed healthcare providermay be required before services can be rendered.
Scope & Purpose of Massage Therapy
Massage therapy is intended to promote relaxation, reduce muscular tension, improve circulation, and support general wellness.
Clients should understand that:
Massage therapy is not a substitute for medical examination, diagnosis, or treatment
Massage therapists do not diagnose medical conditions, prescribe medications, perform spinal or skeletal adjustments, or treat disease
Any information shared by the therapist is for educational purposes only
Clients are responsible for seeking care from a qualified healthcare provider when appropriate
Clients are encouraged to communicate openly during sessions and to notify the therapist immediately if discomfort, pain, dizziness, or any adverse reaction occurs.
Insurance, HSA & FSA
Massage therapy services may be eligible for reimbursement under certain insurance, HSA, or FSA plans when medically necessary. Coverage is determined by the individual plan, not the provider.
Clients are solely responsible for verifying benefits, coverage requirements, and documentation needed for reimbursement.
Professional Conduct & Ethical Standards
All services are provided in compliance with South Carolina licensure laws and professional ethical standards, including proper draping procedures to ensure client comfort, privacy, and modesty at all times.
Any behavior, communication, or request of a sexual, inappropriate, or abusive nature will result in immediate termination of the session. The therapist reserves the right to end any session at their professional discretion, and future appointments may be declined.
Assumption of Risk & Health Disclosure
Clients acknowledge that massage therapy involves inherent risks. Full and honest disclosure of health information is required, and clients assume responsibility for outcomes related to undisclosed conditions or failure to follow professional guidance.
Appointment Length & Punctuality
Appointments include 50 or 80 minutes of hands-on therapy, based on the service booked, provided the client arrives on time.
Late arrivals may result in a shortened session
Full session fees apply regardless of reduced treatment time
Arrivals 15 minutes or more past the scheduled start time are considered a no-show and charged in full
Cancellations & No-Shows
A minimum of 24 hours’ notice is required to cancel or reschedule an appointment. Late cancellations and missed appointments are charged the full session fee.
Age Requirement
Clients must be 18 years of age or older to receive massage therapy services, or parent must be present during entire session.
